Position Title: Case Manager II (Benefits Lead)
Position Classification: Full-Time, Non-Exempt
Reports to: Director of Client Services
Wage Rage: $14.25 – 16.00 per hour
Duties & Responsibilities
The Case Manager II is responsible for providing case management services to clients as an integral component to the interdisciplinary client-centered service delivery model. Programmatic leadership is provided by the Director or Client Services to achieve specific functions that include:
Provide case management services in collaboration with the clients’ Interdisciplinary team, e.g. Physician, Primary Social Worker, Nurse Case Manager, Psychotherapists, Volunteers, Nutritionist, Family/Support System, and client.
Taking the lead role on the Interdisciplinary Team on all issues related to monolingual-Spanish speaking clients.
Interview HIV+ clients and perform evaluations of presenting problems and issues; prepare case history and treatment plans; assign volunteers as appropriate.
Provide ongoing case management services to clients through referral and liaison with appropriate community resources, screening, supervision and support of SCAP volunteers, ongoing needs assessment and counseling of clients and their family systems.
Develop and maintain linkages with hospitals, mental health services, criminal justice system, social service agencies and individuals; networking to assist clients in accessing or maintaining benefits.
Maintain client charts, program manuals, files, and logs, in a professional, accurate, and timely manner to accurately document service delivery activities.
Participate in trainings and in-services for emotional and practical support volunteers and other client service volunteers as directed.
Provide telephone counseling and community referrals as requested. Provide on-call social work services during regularly scheduled hours as arranged.
Attend and participate in co-case management meetings and other community meetings as required.
Provide additional support services to HIV-affected clients/community as directed, including benefits planning, family/friends meetings, facilitation of support groups, and other duties as assigned. Services are provided on-site at the agency, in clients’ homes, and in a variety of community locations throughout the county.
Work co-operatively with supervisor and other staff to establish and develop programs, services and policies that effectively serve client population.
Perform other related duties as assigned.
Qualifications
B.A. or equivalent job experience
Bilingual/Bicultural capacity
Experience working within monolingual Spanish speaking communities. Knowledge of and sensitivity to the specific socio-cultural and economic factors present in the Latino community, with an understanding of how disenfranchisement affects/challenges this population.
Thorough knowledge of the psychological and social aspects and characteristics of emotional disturbances and mental illness; and principles and methods of counseling, case-management.
Working knowledge of the psychological and social issues related to HIV disease; substance abuse and recovery issues; the issues faced by the mentally ill and socially disturbed; community resources; the laws, policies and procedures of health and social service agencies; and clients’ legal rights.
Ability to assess behavioral, psychosocial and emotional problems and develop and implement treatment plans accordingly; establish and maintain effective and professional working relationships with clients, their families, agency staff and volunteers and personnel of other community agencies; conduct effective individual and group counseling; prepare reports, maintain case records, and accurately interpret data; lead and work effectively as part of a treatment team; present ideas, facts and recommendations effectively, orally and in writing.
Word Processing and Database/spreadsheet skills preferred. Knowledge of Filmmaker Pro helpful.
Excellent organizational skills, ability to work both independently and on a team; ability to work well under stressful conditions, ability to relate well to individuals with a wide range of life experiences and backgrounds.
Possession of a valid Class C driver’s license and an insured vehicle is required.

Managing the agency’s accounting/bookkeeping systems and vendors, including A/P, A/R, payroll, financial reporting, and fiscal agency activities; compliance with local, state and federal requirements; supervising the bookkeeper.
Managing the agency’s personnel systems, including personnel files, payroll, vacation and sick leave accruals, and benefits administration.
Contract management.
Facilities management, including premises, computer network and other office equipment.
Assist Executive Director in developing and monitoring organizational and project budgets.
Serve as member of management team.
Participate and assist Executive Director, Education and Prevention Director and Client Services Director in program planning and budget processes.
Calendar and report MAA and other reports to Fiscal Agent, County, foundations and other donors.
Qualifications
Minimum 3 years non-profit management experience.
Comprehensive administrative skills.
Comprehensive accounting skills.
Staff supervision skills
Contract management experience.
Effective oral and written communication skills.
Computer literacy, including word-processing, spreadsheet and accounting software.
